Jo, aged 17, had been in care since he was 11 years old. He was preparing to leave care but was worried about his pathway plan. Jo had some problems with his health, but his plan didn’t include an action plan for how his needs would be met once he left care. Jo rang the Children’s Legal Centre for advice.
